Home Health Case Manager

We are seeking a dedicated professional to oversee and manage in-home care services, conduct comprehensive assessments, develop care plans, and coordinate with healthcare providers.
The ideal candidate will have strong social work and case management skills, experience in utilization management and behavioral health, knowledge of home care practices, and excellent organizational and communication skills.
Responsibilities include:
- Clinical assessment and care planning for clients
- Coordination with healthcare providers and community resources
- Caseload management and prioritization
- Documentation and reporting of client progress
To succeed in this role, you will require a Bachelor's degree in Social Work, Nursing, or a related field, and previous experience in a clinical care or management position.
