Job Description

This is a full-time hybrid role for a Clinical Care Manager, with some work from home flexibility. The position oversees and manages in-home care services, conducting comprehensive assessments, developing care plans, and coordinating with healthcare providers.

Responsibilities include case management, utilization management, and providing support to ensure the highest quality of care for clients. Collaboration with team members and clients is necessary to monitor and adjust care plans as needed.
