About the Role:This leadership position involves coordinating and overseeing the delivery of high-quality care services within our dynamic team. As a key member, you will develop and implement effective case management models, monitor client outcomes, and ensure service effectiveness.
Key Responsibilities:- Develop and coordinate the Home Care Package Program in collaboration with senior management.
- Create and execute a case management model of care, monitoring client success and service efficacy.
- Ensure adherence to budget guidelines for package expenditure.
- Contribute to strategic marketing efforts to enhance growth in Home Care Packages.
- Identify and implement improvements to elevate care quality and competitiveness.
- Nurture relationships with key stakeholders, including government agencies, healthcare providers, and community organizations.
- Maintain data collection requirements and funding targets.
- Effectively manage compliments, complaints, and feedback to drive continuous improvement.
- A minimum of 3 years' experience in aged care, home care, or a related field.
- In-depth knowledge of the Home Care and/or Aged Care industry, including the Aged Care Quality Code of Conduct, Aged Care Standards, and accreditation processes.
- Excellent communication skills with the ability to build rapport with internal and external stakeholders.
- Experience in budgeting and fund management related to Home Care Packages.
- Ability to travel intra and interstate.
- Willingness to participate in after-hours on-call duties.
