We are seeking a skilled Nurse Practitioner to join our team in Community Palliative Care. The successful candidate will be responsible for providing expert clinical support, focusing on palliative care and aged care assessment skills.
Key Responsibilities
- Assess, investigate, and implement appropriate treatment for patients within advanced nursing practice guidelines.
- Provide expert clinical support to the palliative care team, including palliative care and aged care assessment skills.
- Facilitate timely patient review and manage patients with palliative/end of life care needs.
- Develop and deliver education programs to enhance nursing practice through professional development.
Requirements
- Registered Nurse with a commitment to ongoing learning and professional development.
- At least 5000 hours of advanced nursing/midwifery practice in the last six years, or working towards this within the next three years.
- Demonstrated clinical competency in supportive and palliative care, with a specific interest in aged care.
- Able to work independently, assessing, investigating, and implementing treatment plans for patients.
- Effective communication and interpersonal skills, with the ability to work sensitively with patients, carers, and healthcare teams.
