We are seeking a healthcare professional to join our team in providing medical care to children, youth, adults, and seniors in their homes and other settings in communities across Sydney.

Key Responsibilities:
  • Patient assessment and treatment planning
  • Medication management for chronic conditions such as diabetes and dialysis
  • Providing wound care services including negative pressure wound therapy and complex wounds
  • Supporting patients with palliative care needs
Requirements:
  • Licensed as a Licensed Practical Nurse in the province of practice
  • Experience in community health is an asset
  • A valid driver's license and access to a reliable vehicle for home visits
  • A current and original copy of a satisfactory Criminal Records Check
  • Ability to wear Personal Protective Equipment
Working Conditions:

This role requires a detail-oriented approach in a dynamic environment. The successful candidate will be required to walk, sit, stand, and climb stairs throughout the day, with some tasks requiring fine hand movements.
