Stepped Care Coordinator Job Summary

This role involves providing direct support to clients in navigating their care journey, ensuring a seamless experience at every step.

The ideal candidate will have extensive experience in mental health and/or AOD settings, including knowledge of systems, legislation, and brief-intervention approaches.

  • A strong background in client engagement and brief interventions is essential for this position.
  • Candidates with leadership skills to manage client care and team coordination will be highly regarded.
  • Experience working independently and as part of a care team in community-based recovery models is required.

Requirements

  • Qualifications in psychology, social work, mental health nursing, counselling or a related field are preferred.
  • Maintained relevant annual registration and/or membership of professional body is desired.
