Job Title: Stepped Care Coordinator

  • Manage Care Processes: Oversee collaborative care planning, transitions between services, and aftercare.">
  • Strengthen Partnerships: Build relationships with local hub partners and the lived experience workforce to foster community-driven care.">

The ideal candidate will have:

  • Experience: Extensive background in mental health and/or AOD settings, including knowledge of systems, legislation, and brief-intervention approaches.">
  • Strong leadership: You will take charge of client care and team coordination, ensuring clients receive the best possible outcomes.">
  • Community-focused: Experience working independently and as part of a care team in community-based recovery models.">
  • Collaboration Skills: Proven ability to engage and collaborate with diverse services and professionals in the mental health space.">

Qualifications

  • Qualifications in psychology, social work, mental health nursing, counselling or a related field.">
  • A maintained relevant annual registration and/or membership of professional body (preferred).">
