
Palliative Care Nurse

This role offers a unique opportunity to provide comprehensive care and support to patients and their families through the palliative care journey. The successful candidate will be part of an experienced, multidisciplinary team that delivers exceptional care in acute, community, and residential settings.
Key responsibilities include providing nursing care to patients referred to the consultation service, as well as clients at home or in residential aged care facilities. This involves working collaboratively with the team to coordinate care and supporting patients and their families through end-of-life care, including bereavement support and advanced care planning.
The Specialist Palliative Care Service operates primarily Monday to Friday from 08:30 to 17:00, with participation in a rotating after-hours on-call roster (17:00 to 20:30). Staff may also be rostered to cover a day shift on weekends approximately every 4–5 weeks, depending on leave and service needs.
As a dedicated RN, you will work closely with the team to deliver dignified and compassionate care that meets the individual needs of each patient. This includes coordinating care across different settings and ensuring seamless transitions for patients and their families.
