Job Title: Family Care Support Case Manager
About the RoleThe EIS Case Manager provides initial screening and assessment, developing family case plans with carers. They identify barriers and provide practical in-home support, overseeing case management activity and offering parenting and trauma information for carers.
The Early Intervention Services (EIS) program works with parents and family carers to improve safety and connectedness with local community services. This role is part of the EIS and reports to the Coordinator Pilbara Service.
This is a Permanent Full-Time opportunity based in South Hedland, WA within our Intensive Family Support Services/ Family Care Support Services Program.
Key Selection Criteria- A knowledge and understanding of Aboriginal culture and values.
- An appropriate qualification in human services or equivalent experience and skills.
- An understanding of attachment, trauma theories, child development, child-centred approach, and resilience.
- Demonstrated commitment to working collaboratively with community services, other agencies, and the community.
- An understanding of the child protection and out-of-home care service system.
- Hourly Rate of $51 to $53.31 as outlined in the SCHADS Award Level 5.
