Family Care Navigator

We are seeking a skilled professional to join our team as a Family Carer Support Case Manager.
The successful candidate will be responsible for providing initial screening and assessment, developing personalized family case plans with families. They will identify barriers and provide in-home support for carers, oversee case management activity, offer parenting and trauma information, and specific supports for children to strengthen links to community and culture.
This role is an integral part of our Early Intervention Services (EIS) team and reports directly to the Coordinator Pilbara Service.
Key Qualifications and Skills- A deep understanding of Aboriginal culture and values, as well as awareness of current issues faced by Aboriginal children, young people, and their families.
- An appropriate qualification in human services or equivalent experience and skills.
- An understanding of attachment, trauma theories, child development, child-centered approach, and resilience.
- Demonstrated commitment to working collaboratively and liaising with community services, other agencies, and the community.
- An understanding of the child protection and out-of-home care service system.
