Clinical Care Manager

At our residential aged care home in Ballarat, VIC, we are seeking a passionate and experienced Clinical Care Manager to lead and manage clinical teams ensuring high-quality care delivery for residents.

  • Key Responsibilities:
  • Develop and implement tailored care plans to meet the individual needs of residents promoting optimal physical and emotional well-being.
  • Collaborate with multidisciplinary teams including physicians, physiotherapists, and social workers to provide comprehensive care.
  • Promote a culture of continuous improvement fostering a supportive and compassionate work environment.
  • Ensure compliance with all regulatory and quality standards related to medication administration, infection control, and patient safety.

Requirements:

  • Registered Nurse (RN) qualification with current nursing registration.
  • Minimum 5 years of experience as a registered nurse with significant experience in aged care settings.
  • Demonstrated leadership and change management skills with experience leading a clinical team.
