Life Changing Case Manager Opportunity

We are seeking a skilled Case Manager to join our team. This role involves delivering outreach support to clients in the community and building strong referral relationships with other stakeholder organisations.
About the Role:

- You will be working closely with clients to understand their needs and develop individualised plans that address their unique circumstances.
- The successful candidate will have excellent communication and interpersonal skills, as well as the ability to work effectively in a team environment.
Key Responsibilities:
To provide comprehensive case management services to clients with complex needs, including:
- Providing regular check-ins and progress updates;
- Collaborating with other service providers to ensure seamless support for clients;
- Developing and implementing individualised plans that align with client goals and aspirations;
To build and maintain effective relationships with stakeholders, including:
- Government agencies;
- Non-profit organisations;
- Healthcare providers;
Requirements:
- Relevant tertiary qualifications in Social Work, Community Development, Community Services, Criminology, Law, Psychology, Counselling or equivalent;
- Demonstrated experience in case management, particularly with marginalised populations;
- Strong knowledge of relevant laws, policies, and procedures related to social work and community development;
