Aboriginal Care Coordination Specialist

We are seeking a dedicated and compassionate Aboriginal Care Coordination Specialist to join our team at the Elders Council of Tasmania Aboriginal Corporation.
As a key member of our team, you will play a vital role in co-ordinating the delivery of Home and Community Care services to maintain the independence and dignity of Aboriginal frail and aged individuals in their own homes.
Key Responsibilities- Co-ordinate services to support clients in maintaining their independence and quality of life;
- Maintain strong community connections and work collaboratively with key stakeholders to develop and maintain services that meet the needs of our clients;
- Identify and address client needs through effective assessment and care planning;
- Collaborate with other service providers to ensure comprehensive and coordinated care;
- Develop and maintain accurate records to ensure the high-quality delivery of services.
This is a full-time position working 38 hours per week. The successful applicant will be based at our Hobart office, but may be required to travel to other locations as part of their role.
The salary for this position is approximately $65,000 - $75,000 per annum, depending on experience.
