The role of a Support Coordinator involves connecting participants with NDIS plans to community supports and services. The ideal candidate will have strong communication skills, leadership abilities, and problem-solving expertise.
- Help members understand their NDIS plan and manage resources effectively.
- Work collaboratively with others to achieve goals and outcomes.
- Monitor progress, outcomes, and quality of services and supports.
- Provide reports to the NDIS and assist with review meetings.
- Valid NSW Driver's License and access to a vehicle.
- Satisfactory NDIS worker screening check and Working with Children's check.
- Bachelor's degree in social science, human sciences or equivalent.
- Minimum 2 years relevant work experience.
