We are seeking an experienced professional to fill a Coordinator of Supports role, empowering individuals with disabilities to build skills and achieve their goals under the NDIS.
- Empower people living with disabilities to build skills and connect with their community
- Coordinate services that promote independence, inclusion, and choice - delivered at home and in the community
The ideal candidate will possess strong understanding of person-centred and strengths-based practice, excellent communication skills, and ability to work independently. Key qualifications include experience supporting adults and/or children with disabilities, familiarity with NDIS processes and disability-related legislation, and current Australian Driver's Licence.
Key Responsibilities:- Coordinate supports and life transitions
- Support employment, community access, and independent travel
- Build daily living and life skills with clients
- Provide early intervention where needed
