Job Title: Support Services Coordinator

About the Role:

We are seeking a highly skilled and compassionate Support Services Coordinator to empower individuals with disabilities to achieve their goals and build skills in daily living and life skills. This is an exciting opportunity to work directly with clients and families, coordinating services that promote independence, inclusion, and choice.

The ideal candidate will have strong understanding of person-centred and strengths-based practice, excellent written, verbal, and interpersonal communication skills, as well as the ability to work independently, manage time, and meet deadlines.

Duties and Responsibilities:

  • Coordinate supports and life transitions for individuals with disabilities
  • Support employment, community access, and independent travel opportunities
  • Build daily living and life skills with clients, including early intervention where needed
  • Work within established guidelines and budgets to deliver quality services
  • Develop strong relationships with local services and networks to ensure seamless support for clients

Requirements:

  • Qualifications or experience in Disability, Social Work, Education, or related field
  • Experience supporting adults and/or children with disabilities
  • Strong understanding of person-centred and strengths-based practice
  • Excellent written, verbal, and interpersonal communication skills
  • Ability to work independently, manage time, and meet deadlines
  • Confident with Microsoft Office and reporting software
