Job Description:
- Conduct thorough assessments using My Aged Care to identify the needs and goals of clients.
- Provide referrals to Commonwealth-funded services to ensure seamless transitions in care.
- Offer vital information to clients and their carers to empower them with knowledge and confidence.
Essential Requirements:
- Applicable vaccination requirements for a Category A role.
- Current AHPRA Registration as a Registered Nurse or Enrolled Nurse (Division 1 or 2) under the Nurses Act 1990 (NSW).
