We are seeking a highly motivated and compassionate Support Coordinator to join our Remote Support Coordination team at Breakthru. As a Support Coordinator, you will play a vital role in empowering individuals to navigate the NDIS, understand and maximise their NDIS plan, engage with other services and supports, and ensure plan outcomes and individual expectations are met.
Key Responsibilities- Provide personalised guidance and understanding to help individuals become confident in managing their NDIS package and associated services.
- Understand individual plans and funding available, and find service options that best meet unique needs and outcomes.
- Track progress and demonstrate individual success through goal tracking.
- Develop skills in others to enable them to become independent and self-assured in managing their NDIS funding.
We are looking for a person who is genuinely thrilled to see others achieve their goals, can easily adapt their communication style to build relationships, and appreciate individual pace - noticing and celebrating key milestones and the small steps along the way.
What We Offer- A permanent part-time position with a competitive salary package, including a base salary range of $65-70K + Super + Salary Packaging + Royal Rewards, pro rata'd.
- Relevant certificates or qualifications with experience, attract the higher rate.
- A laptop and phone, as well as access to a knowledgeable senior leadership team, including a Practice Lead in Support Coordination to guide you.
- A thorough induction and orientation to support your success.
