Dementia Support Specialist

About the Role
A rewarding opportunity for passionate Dementia professionals to join the Dementia Support Australia team at HammondCare on a fixed-term full-time contract based out of our Daw Park site.
Key Responsibilities
- Work collaboratively with peers to develop the skills, expertise, and/or applied knowledge of aged care staff and carers to understand the person living with dementia and develop best practice support and care.
- Support the implementation of recommendation plans on the ground, drawing on modelling and coaching techniques, and seek guidance from Team Leader, Dementia Consultant, and Senior Consultant as required.
- Facilitate and support the trial and implementation of brokerage items with clients and their carers, and seek guidance from Team Leader, Dementia Consultant, and Senior Consultant as required.
Requirements
- Strong organisational abilities, including effective time management and ability to prioritise in a busy work environment and competing demands.
- The ability to work both independently and as part of a team.
- Demonstrated commitment to excellence in customer service principles, including conflict resolution, to ensure the consistent provision of exceptional service to internal and external customers.
