About the Role
A unique opportunity for passionate dementia professionals to work with HammondCare, a leading provider of aged care services, on a fixed-term full-time contract.
The Dementia Support Coach will provide on-the-ground support, implement relationship-based support strategies, and model and coach clients, carers, and care staff.
- Collaborate with peers to develop the skills and expertise of aged care staff and carers to understand the person living with dementia and provide best practice support and care.
- Support the implementation of recommendation plans, drawing on modelling and coaching techniques, and seek guidance from Team Leader, Dementia Consultant, and Senior Consultant as required.
- Facilitate and support the trial and implementation of brokerage items with clients and their carers, and seek guidance from Team Leader, Dementia Consultant, and Senior Consultant as required.
What We Need from You
- Strong organisational abilities, including effective time management and ability to prioritise in a busy work environment.
- The ability to work both independently and as part of a team.
- Demonstrated commitment to excellence in customer service principles, including conflict resolution, to ensure the consistent provision of exceptional service to internal and external customers.
