About the Role
A unique opportunity has arisen for a highly skilled Dementia professional to contribute to the Dementia Support Australia Team at HammondCare. This Fixed Term Full-Time Contract until June 2026 offers a chance to make a meaningful impact on the lives of individuals living with dementia and their carers.
Key Responsibilities
- Provide expert consulting and advisory services to support the delivery of Dementia Support Australia.
- Develop the skills and expertise of aged care staff and carers to understand the needs of individuals living with dementia.
- Develop and implement best practice support and care strategies to reduce the impact of behaviours on individuals living with dementia.
- Assess clients with behaviours related to dementia and provide resources and ideas for effective care provision.
- Stay up-to-date with current evidence-based guidance and knowledge to ensure alignment with the Dementia Centre.
- Plan engagement with individuals and teams to achieve agreed practice development objectives and quality improvements.
- Contribute to other projects and assignments as required, leveraging areas of skill, expertise, and knowledge.
- Understand and engage with HammondCare's Mission in Action, Model of Care, Case Management Framework, and practice guidelines, policies, and procedures.
Requirements
- Expertise in supporting individuals living with dementia.
- Experience in Australian residential aged care and/or community aged care.
- High-level negotiation, communication, and relationship-building skills.
- Excellent problem-solving and decision-making skills.
- Ability to prioritise multiple tasks and meet deadlines.
- Excellent written and verbal communication skills.
- Excellent stakeholder management.
- A current Australian driver's licence and willingness to travel.
- Empathy and patience.
- Relevant qualification in Allied Health, Nursing, Aged Care, or Mental Health is essential.
