About the RoleThe successful candidate will be responsible for managing and coordinating care services, developing and implementing care plans tailored to individual client needs, and liaising with clients, families, and healthcare professionals. They will also foster strong relationships with clients, stakeholders, and team members, while assisting clients in meeting their unique needs and goals.
Key Responsibilities- Manage and coordinate care services to ensure high standards of care
- Develop and implement care plans tailored to individual client needs
- Liaise with clients, families, and healthcare professionals to ensure comprehensive care
- Foster strong relationships with clients, stakeholders, and team members
- Ensure compliance with relevant legislation and organizational policies
- Monitor and evaluate care services to continuously improve quality and efficiency
We offer a competitive salary of $90,000 - $110,000 per annum, depending on experience, plus a range of benefits including 5 weeks annual leave, annual leave loading, and optional salary packaging.
Requirements- Demonstrate experience in case management and client care coordination in a Community Aged Care setting
- Demonstrate strong verbal, written, and interpersonal communication skills
- Show ability to develop staff, foster teamwork, and effectively work as a team member
- Provide proof of immunity or willingness to become vaccinated against certain diseases
- Hold a current Queensland driver's licence and provide tertiary qualifications and/or relevant experience in a health-related field
