We are seeking an experienced Occupational Therapist to join our team as a Care Coordinator for rural communities in Rockhampton.
The successful candidate will play a vital role in delivering high-quality, client-centered care to individuals and families in need.
About the Role
As a Care Coordinator, you will be responsible for conducting comprehensive assessments, developing individualized treatment plans, and implementing therapeutic interventions to support clients' independence and quality of life.
You will work collaboratively with a multidisciplinary team to ensure holistic care and stay current with industry best practices through ongoing education and training.
Key Responsibilities
- Conduct thorough assessments of clients' physical, emotional, and cognitive needs.
- Develop tailored treatment plans and goals that cater to each client's unique requirements.
- Implement evidence-based therapeutic interventions and monitor progress.
- Educate clients and their families on strategies and techniques to enhance independence and overall well-being.
- Maintain accurate and timely documentation of client progress and treatment plans.
- Collaborate with other healthcare professionals to ensure comprehensive care.
Requirements
- Bachelor's or Master's degree in Occupational Therapy from an accredited institution.
- Current registration with AHPRA or eligibility to apply (completed stage 1 desktop assessment).
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a team.
- Compassionate and patient-centered approach to therapy.
