Care Coordinator Healthcare Professional

We are seeking a skilled Care Coordinator to join our team at Right at Home Central Queensland. This is a permanent part-time position based in the Rockhampton Office.
In this role, you will be responsible for providing high-quality client-focused care in a community setting. You will work under the supervision of a care manager and have the opportunity to develop your skills and expertise in a supportive and collaborative environment.
Key responsibilities include:
- Interpreting health, aged, and disability care policies and regulations for carers and nursing staff
- Ensuring compliance with local, state, and federal regulatory agencies related to aged and disability care services
- Implementing and maintaining operating systems, including Visual Care, policies, and procedures for caregivers, nurses, and other care professionals
- Coordinating caregiver management to meet 'best practice' standards, including encouraging ongoing professional development of the care team
- Liaising with the accounts department to ensure proper billing in line with the care plan
To be successful in this role, you will require:
- A current registration with AHPRA
- Satisfactory Australian National Police check
- Current or willingness to obtain a first aid and CPR certificate
- Professional Indemnity Insurance
- A smart mobile phone (iPhone/Android)
- Driver's licence and reliable presentable vehicle
- COVID-19 vaccinations
The estimated salary for this position is $60,000 - $80,000 per annum, depending on experience. We offer a competitive remuneration package, opportunities for professional growth and development, and a positive work environment.
