About the Role

As a Care Navigator, you will work collaboratively with consumers to connect them with services and supports in the community, facilitating person-centred and integrated care.

  • This role delivers practical support through information sharing and education, identifying and addressing barriers to access and facilitating warm referrals, ensuring consumers are linked to the right service at the right time.
Key Responsibilities
  • Work collaboratively with consumers to identify their needs and develop tailored plans for care and support
  • Facilitate connections with services and supports in the community, promoting seamless transitions and coordinated care
  • Provide education and support to consumers and their families, empowering them to make informed decisions about their care
  • Collaborate with multidisciplinary teams to ensure comprehensive and holistic care
Requirements
  • Tertiary qualification in a relevant discipline, or extensive experience in mental health, health, community services or social sciences
  • Demonstrated understanding of the health and wellbeing needs of consumers with mental health and/or substance use problems
  • Demonstrated ability to engage with consumers and their families to provide care navigation and referral support
  • Current driver's license and willingness to travel for the role
