Crisis Accommodation Case Manager

CatholicCare NT is a not-for-profit organisation dedicated to creating safe, respectful and empowered communities across the Northern Territory.
As a Crisis Accommodation Case Manager, you will play a pivotal role in supporting individuals experiencing crisis accommodation needs at Crerar Road. Working collaboratively with our multi-disciplinary team, you will facilitate referrals, promote housing stability and wellbeing, and encourage participation in site activities.
To succeed in this role, you will possess:
- A relevant tertiary qualification in social work, community services, welfare or a related field – OR equivalent experience in crisis, homelessness or case management support
- Strong case management, assessment and planning skills
- Effective communication, culturally responsive practice and advocacy skills
- Demonstrated ability to work collaboratively in a diverse, multi-disciplinary team
