Case Management Specialist

Deliver timely and effective case management services to individuals experiencing crisis accommodation needs.
- Provide intake and assessment services, identifying the client's needs and developing a personalized plan to address their requirements.
- Collaborate with the multi-disciplinary team to support housing stability and wellbeing, facilitating referrals and promoting participation in site activities.
The ideal candidate will possess strong case management, assessment, and planning skills, as well as effective communication and culturally responsive practice abilities.
Required Skills and Qualifications- Relevant tertiary qualification in social work, community services, welfare, or a related field – OR equivalent experience in crisis, homelessness, or case management support.
- Strong case management, assessment, and planning skills.
- Effective communication, culturally responsive practice, and advocacy skills.
- Demonstrated ability to work collaboratively in a diverse, multi-disciplinary team.
- Above Award Salary.
- Six Weeks Annual Leave plus Two Weeks Personal Leave.
