
Life Changing Rehabilitation Support Specialist

This position plays a vital role in supporting individuals with treatment, care and support needs as they transition back into their community following serious injury.
The successful candidate will have an allied health background or a relevant degree in a health-related discipline or rehabilitation advisor. They will be equipped with transferable skills to coordinate rehabilitation services to people with disabilities similar to those supported by our schemes.
- Key Responsibilities:
- Coordinate rehabilitation services to meet the needs of individuals with disabilities.
- Determine treatment planning needs and coordinate relevant services and support.
The ideal candidate will possess practical experience in managing a caseload of participants, using critical reasoning skills to ensure a high degree of integrity in the decision-making process. They will also utilise excellent communication skills to build rapport and connect with individuals, their families and their rehabilitation team.
This role involves working collaboratively with internal specialist services, external case managers, health professionals and other key stakeholders to coordinate the most effective options to meet the treatment, care and support needs of individuals.
Required Skills and QualificationsTo be successful in this role, you will need:
- A strong background in allied health or a relevant health-related discipline.
- Prior experience in managing a caseload of participants and coordinating rehabilitation services.
- Critical reasoning and problem-solving skills.
- Excellent communication and interpersonal skills.
