Complex Case Manager Position

About the Role:

This is a unique opportunity to join our team as a Clinical Complex Case Manager. Your primary responsibility will be assessing and managing the needs of Military Veterans and their families.

You will work collaboratively with an established national team of clinical and non-clinical staff to provide short-term case management support to clients with complex health and psychosocial needs. This includes identifying and responding to risk on an ongoing basis, engaging with clients to develop responsive case plans, and liaising with internal and external stakeholders to establish strong support networks.

In this role, you will also respond to urgent client needs and assist with phone duties on a rotating basis.

Key Responsibilities:
  • To assess and manage the needs of Military Veterans and their families.
  • To identify and respond to risk in a timely manner.
  • To engage with clients to develop responsive case plans.
  • To liaise with internal and external stakeholders to establish strong support networks.
  • To respond to urgent client needs and assist with phone duties.
Requirements:
  • A qualification as a Social Worker, Psychologist, Mental Health Registered Nurse, or Occupational Therapist with registration (or eligibility) with the relevant professional body.
  • Frontline experience in assessing and responding to clients with complex mental health presentations.
  • Experience in short-term case management of a caseload of clients with complex health and/or multiple psychosocial needs.
  • Strong time management skills, flexibility, and responsiveness in a fast-paced environment.
  • Australian Citizenship.
