
Community Care Specialist

We are seeking a highly skilled Case Manager to join our team in the Wollongong & Illawarra region. This role involves collaborating closely with children, young people, their families and carers to create and implement case plans that achieve permanency goals.
As a Case Manager, you will build and maintain strong relationships with the individuals we support, ensuring every voice is heard and every need is met. Your efforts will be pivotal in supporting and monitoring placements, guiding carers in their foster care journey and helping them implement effective strategies that best support the children and young people within their care.
To be successful in this role, you will have a relevant bachelor's degree such as Social Work, Social Welfare, Psychology or similar, and previous case management experience within an Out of Home Care (OOHC) /Child Protection setting. You will also have experience supporting Children & Young People with disability, who have experienced backgrounds of trauma, abuse and/or neglect.

- Working understanding of the NSW Permanency Support Program
- Understanding of trauma and its effects on children and young people
