Job Title:

Care Coordinator Specialist

We are seeking a highly skilled Care Coordinator Specialist to join our team. As a Clinical Nurse Specialist Grade 2, you will play a vital role in providing high-level, person-centred, evidence-based clinical nursing care and embracing a practice development philosophy in its delivery.

  • Comprehensive nursing assessments and referrals;
  • Expert consultation and educational support to staff, patients, and family in the cancer setting;
  • Evaluation and supervision of complex nursing care in the oncology area;
  • Assessment and management of cancer patients within specified tumour groups.

The successful candidate will have excellent communication and interpersonal skills, with the ability to work autonomously within a multidisciplinary team. You will be responsible for collaborating with tumour stream CNCs to ensure safe management of relevant patient groups and providing leadership and mentorship to less experienced clinicians.

Key Responsibilities:

  • To provide comprehensive nursing care and assessments for cancer patients;
  • To develop and implement individualised care plans for patients;
  • To collaborate with other healthcare professionals to ensure effective patient care;
  • To supervise and develop less experienced clinicians.

Requirements:

  • Degree in Nursing or related field;
  • Registration as a Nurse Practitioner or Clinical Nurse Specialist;
  • Extensive experience in oncology nursing;
  • Excellent communication and interpersonal skills.
