Community Care Specialist

This is a dynamic role for an experienced Registered Nurse who uses evidence-based practice to deliver person-centered care in collaboration with individuals and a multidisciplinary healthcare team.

As a Community Care Specialist, you will provide supervision and support for less experienced clinicians to achieve high-quality patient outcomes.

Key responsibilities include:

  • Providing nursing care that is centered around the individual's needs and preferences.
  • Collaborating with other healthcare professionals to ensure seamless care delivery.
  • Supervising and supporting less experienced clinicians to enhance their skills and confidence.
  • Using evidence-based practice and critical thinking to advocate for and implement individualized care plans.
  • Effective prioritization and organization of workload to deliver quality care.

Selection criteria includes:

  • Current registration with AHPRA as a Registered Nurse.
  • Demonstrated competencies in providing nursing care in a multidisciplinary team setting.
  • Excellent clinical communication and interpersonal skills.
  • Application of evidence-based practice and critical thinking to advocate for individualized care.
  • Able to prioritize and organize workload effectively.

Additional requirements include:

  • Occupational assessment, screening, and vaccination against specific diseases.
  • Must be an Australian citizen or permanent resident or have a valid visa to live and work in Australia.
