Job Opportunity: Facilitate Services with Compassion and Excellence

Elevate the lives of individuals with disabilities and older Australians by providing exceptional support services as a Service Facilitator. This dynamic role demands a caring and proactive individual who can work collaboratively with clients and staff in various settings.

The ideal candidate will possess excellent coaching and mentoring skills, ensuring high-quality services and supports are delivered to clients. Key responsibilities include:

  • Developing and implementing personalized client goals in collaboration with clients and staff.
  • Managing rosters accurately and processing timely claims for payment.
  • Providing supervision, coaching, and mentoring to direct care staff.
  • Maintaining effective communication and coordination between services, staff, and stakeholders.
  • Assisting clients in managing their finances according to organizational policies and procedures.
  • Embodying the organization's values by understanding each individual, delivering outstanding service, working collaboratively, and driving innovation.
  • Possessing a solid foundation of knowledge in relevant legislation to protect vulnerable individuals from abuse and neglect.

Successful candidates will have:

Essential Qualifications and Skills
  • Experience in a similar role within the aged care sector or related field.
  • Relevant tertiary qualifications, particularly in health-related fields.
  • A commitment to delivering exceptional customer service.
  • A proactive approach to working in a dynamic environment.
  • Proven experience in team supervision and leadership.
  • Excellent interpersonal and communication skills.
  • Ability to work independently and collaboratively within teams.
  • Well-developed administrative and financial management skills.
  • A current driver's license and reliable transportation.
  • First Aid and CPR certification.
  • Competency in Microsoft Office and computer systems.
  • Working With Children Check and NDIS Worker Screen Clearance.
