About the Role

What You'll Bring
  • A strong passion for health and wellbeing, along with excellent relationship-building skills and the ability to thrive in both independent and team-based environments.
  • A relevant qualification, such as Certificate III in Case Management, Aged Care, Disability, Community Services, or a related field.
  • Previous experience in case management within aged care, disability, or youth services.
  • A good understanding of Home Care Packages and the Support at Home program.
  • Excellent communication and interpersonal skills.
  • A valid driver's license and access to a reliable vehicle.
