Job SummaryWe are seeking an experienced Client Services Facilitator to join our team in Ipswich. As a key member of our client services team, you will be responsible for facilitating quality client service delivery for children and young people with disabilities across the Logan, Western Suburbs of Brisbane, and wider Ipswich region.
Key Responsibilities- Client Service Delivery: Facilitate quality client service delivery for children and young people with disabilities, ensuring that their needs are met and their goals are achieved.
- Stakeholder Engagement: Work with relevant stakeholders, including families, carers, and healthcare professionals, to understand individual client support needs and goals.
- Support Worker Management: Work with rosters and recruitment teams to ensure optimum support worker matching, ensuring that clients receive the best possible support.
- Client File Management: Ensure client files are up to date, responding quickly to provide support for situations as they arise.
- Program Design and Planning: Support program and activity design, planning, and resourcing with the Client Services Manager.
- Incident Response: Support and review incidents working with the Quality and Compliance team as required.
- On-Call Roster: Participate in the Client Services Weekend On-Call Roster.
- Experience: At least 2 years' experience in client coordination, preferably in the disability sector and/or with young people.
- Qualifications: Relevant qualification in community services or a willingness to work towards one.
- Skills: Excellent administration skills, comfortable with using digital tools, good decision-making and communication skills, excellent attention to detail, valid Australian Driver's Licence, comprehensively insured vehicle, current First Aid & CPR Certificate, Blue Card, and Yellow/NDIS Worker Screening Clearance Card.
