Client Services Coordinator

Focal Community Services is a well-established provider of supports to people with a disability, their families, and carers. With almost 50 years of experience, we deliver services to people of all ages and needs in their homes and the community.
Our person-centred approach focuses on building capacity, freedom, choice, and control for individuals with disabilities. We work alongside each participant as advocates, helping them maximise and implement all the supports in their plan.
Key Responsibilities:
- Facilitate quality client service delivery for our diverse client base across Southeast Queensland.
- Understand client support needs and goals, briefing relevant stakeholders for each new intake.
- Provide client service information to rosters and recruitment teams for optimum support worker matching.
- Support program and activity design, planning, and resourcing as delegated by the Manager.
- Close follow-up communication with clients and carers as needed.
- Prepare NDIS Progress Reports for plan reviews with the Intake and Quoting team.
- Provide Compliance team with documents and assistance for Incident and Feedback management.
Requirements:
- At least 2 years' experience in client coordination, preferably in the disability sector.
- Excellent administration skills and comfortable with using digital tools.
- Good decision-making skills ensuring effective supports and incident response.
- Valid Australian Driver's Licence and comprehensively insured vehicle.
- Current First Aid & CPR Certificate.
- Blue Card and Yellow/NDIS Worker Screening Clearance Card.
