Job Overview

Deliver Support Services as a Case Manager

  • Provide trauma-informed, culturally safe residential support to individuals at risk of offending or re-offending.
  • Develop and implement community-based programs for relapse prevention, harm reduction, and well-being.
  • Contribute to the formulation and review of resident plans.
  • Build supportive relationships with clients and their families.
  • Ensure the comfort and dignity of residents.
  • Perform assessments and intakes as required.

  • Maintain accurate case notes and documentation.
Requirements

Meet the Essentials:

  • Hold a Certificate IV in Community Services (Alcohol and Other Drugs) or a related qualification.
  • Demonstrate experience working in case management.
  • Bring experience in AOD or therapeutic community settings.
  • Design, facilitate, and participate in AOD and culturally-based group programs.
  • Exceptional communication skills, engaging with diverse clients and stakeholders in a non-judgmental and culturally appropriate manner.
  • Ability to maintain accurate case notes, perform assessments, and deliver trauma-informed, goal-oriented support.
  • Sound understanding of the TC model and responding effectively to challenging behaviors.
  • Strong organisational, time management, and documentation skills.
  • Lived experience and/or Aboriginality (highly desired).
