We are seeking a dedicated Patient Support Specialist to join our team at St Vincent's Private Hospital Melbourne. In this role, you will work closely with patients and their care providers to ensure a smooth transition from hospital to home.
About the Role:
This is an exciting opportunity for a skilled Social Worker to provide patient-focused support in a fast-paced acute and subacute setting. As a Patient Support Specialist, you will be responsible for managing patient discharge plans, coordinating community resources, and collaborating with multidisciplinary teams to deliver exceptional patient care.
Key Responsibilities:
- Develop and implement patient discharge plans that meet individual needs and goals.
- Coordinate community resources to support patient care and well-being.
- Collaborate with medical, nursing, and allied health teams to ensure seamless patient transitions.
- Provide education and support to patients and their families on post-discharge care and management.
Requirements:
- Bachelor of Social Work or equivalent qualification.
- Eligible for registration with the Australian Association of Social Workers.
- Strong knowledge of the Aged Care sector and community resources.
- Excellent communication and interpersonal skills.
