Job Overview

We are seeking skilled professionals to support clients with personal care, community access and daily routines.

  • This role is ideal for individuals who want to make a positive impact in people's lives and provide meaningful care to those in need.

You will work closely with clinicians, case workers, and families to develop and implement personalized care plans that cater to the unique needs of each client.

The ideal candidate will have excellent communication skills, be able to work independently and as part of a team, and possess a strong ability to empathize with others.

Key Responsibilities:

  • Providing personal care and support to clients with complex needs, including behaviors of concern, trauma, forensic backgrounds, and/or high clinical needs.
  • Administering medication and following detailed health support plans.
  • Implementing behavior support plans and using de-escalation techniques to ensure a safe and supportive environment.
  • Collaborating with multidisciplinary teams, including clinicians, case workers, and families, to achieve positive outcomes for clients.

Requirements:

  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Empathy and understanding of the needs of clients with complex requirements.
  • Ability to administer medication and follow detailed health support plans.
  • Knowledge of behavior support planning and de-escalation techniques.
