Dynamic Care Coordinator Opportunity

We are seeking an experienced clinician to join our dynamic team providing transdisciplinary care coordination services in the Emergency Department.

The aim of this role is to reduce presentations / admissions and optimise patient care through the provision of Care Coordination, Rapid Allied Health service and Integrated Care planning.

This opportunity exists for a part-time or full-time Care Coordinator to join our team and work collaboratively with multidisciplinary teams to deliver high-quality patient care.

About the Role

This is an exciting opportunity for a motivated and experienced clinician to join our team and contribute to the development of integrated services for clients with multiple and complex service needs.

  • A commitment to developing integrated services for clients with complex needs
  • An appropriate tertiary qualification and eligible for discipline specific registration
  • A minimum of 2 years clinical experience in a relevant healthcare setting
  • Experience in assessment of people with complex needs, discharge planning, case conference and care planning
  • Experience working in multidisciplinary teams
Key Responsibilities

Deliver high-quality patient care through the provision of Care Coordination, Rapid Allied Health service and Integrated Care planning

Work collaboratively with multidisciplinary teams to develop and implement individualised care plans

Contribute to the development of integrated services for clients with complex needs
