Job Overview

The Manager of Care Coordination plays a pivotal role in ensuring seamless transitions of care and optimizing client experiences. This leadership position involves overseeing a dynamic team to enhance patient outcomes and improve the overall quality of care.

About You

\

We are seeking an experienced professional with a proven track record of developing and maintaining exceptional relationships with internal and external stakeholders, along with outstanding team management skills.

\

You will have a relevant tertiary qualification or extensive experience in Human services, Allied Health, Age Care or Community Service Fields.

\

Knowledge and/or experience in Home Care Packages is highly desirable.

\

Key Responsibilities

\

Ensure service delivery aligns with Aged Care Quality Standards, procedures, policies and supports best outcomes for clients.

\

Lead, manage and mentor the Care Coordination team ensuring they have the required skills, knowledge and capacity to fulfill the requirements of their role.

\

Identify any training needs of the care coordination team.

\
