About the Role
  • This position presents a chance to conduct initial health and social needs assessments, care planning, and case conferencing for newly arrived individuals and families.
  • You will be responsible for coordinating client healthcare by liaising with services, schools, and a range of providers, including facilitating access to interpreters and transportation support.
Key Responsibilities
  • Involve clients and carers in decision-making and collaborate to negotiate agreed care plans.
  • Work closely with local settlement services to quickly address the needs of newly arrived individuals.
  • Promote social connection and integration by referring clients to support and orientation programs.
Requirements
  • AHPRA Division 1 Registered Nurse qualification is essential, with full and unrestricted registration.
  • Minimum of 2-3 years' experience in primary health care settings or as a Refugee Health Nurse is required.
  • A strong grasp of community health, human rights, and trauma-informed care principles is necessary.
  • Demonstrated understanding and respect for diverse cultural backgrounds, traumatic experiences, and unique health challenges are essential.
  • Lived experience and ability to speak a relevant community language is highly desirable, although not essential.
