Job Summary

This is a non-exempt position where the successful candidate will provide case management services to formally homeless disabled individuals living in Permanent Supportive Housing (PSH).

The ideal candidate will have a Bachelor's degree in social, behavioral or human services and at least one year of professionally supervised work in the delivery of mental health, chemical dependency or social services.

We are seeking someone who is motivated, compassionate, and dedicated to helping others achieve their goals and improve their overall well-being.

This role involves working with clients to develop and implement their Individualized Service Plans (ISPs), which aim to maintain housing and meet their identified goals.

The selected candidate will be part of a multidisciplinary team providing services reflective of an integrated service delivery model using evidence-based best practices such as trauma-informed care, motivational interviewing, harm reduction, and IDDT.

The starting wage for this position is $21.00 per hour and regular working hours are 8:00am – 4:30pm, Monday through Friday.

A valid driver's license with less than six points, proof of insurance and access to an operational vehicle are required for this role.
