Case Manager Role

This position involves providing comprehensive case management services to individuals residing in Permanent Supportive Housing (PSH). The primary focus is on developing and implementing personalized service plans, incorporating evidence-based best practices such as trauma-informed care and motivational interviewing.

Main Responsibilities:

  • Creating and executing individualized client goals and objectives within the Individualized Service Plan (ISP)
  • Maintaining housing stability and achieving individual goals, including reducing distress caused by mental health symptoms and substance use issues
  • Fostering improved physical health, increased income, enhanced interpersonal relationships, and increased self-sufficiency and self-determination

This role requires a bachelor's degree in social, behavioral or human services, accompanied by at least one year of professionally supervised work in delivering mental health, chemical dependency, or social services.
