About the Opportunity:

We are seeking a dedicated and compassionate professional to join our team as a Healthy Ageing Liaison Officer. This role will play a vital part in fostering connections between our organisation, community organisations, and health services to enhance the quality of life for older adults in our community.

Key Responsibilities:

- Foster relationships with internal and external stakeholders to promote partnerships and extend service reach

- Develop and maintain relationships with service providers to improve access to funded services

- Provide support to residents who require personalised information and practical assistance

- Assist with the development, promotion, and implementation of strategies to increase resident participation and reduce social isolation

Requirements:

- Tertiary qualification in Allied Health or related discipline and/or relevant experience in Aged Care, Aged Care Assessments, or the Community Services sector

- Proven ability to communicate and work with diverse people experiencing vulnerabilities

- Demonstrated understanding of Microsoft Office suite and literacy in utilising other digital channels and platforms

- An understanding of the Aged Care system and reforms
