**About the role**
An exciting new opportunity to join our thriving community health care organisation and work within a truly supportive and values-based culture and engaged workforce.
We are now seeking an Access to Community Coordinator to coordinate our social prescription program. The Coordinator will work closely with the Volunteer Coordinator; and Volunteer Community Connectors to facilitate supportive connections with people under 65 that are experiencing isolation or loneliness. The Coordinator will perform intake, assessment and review of clients, and support their engagement in and exit from the program. They will support volunteers and clients to have successful relationships that lead to improved community connection within the areas of Boroondara, Yarra and Manningham.
The Coordinator will be actively involved in stakeholder engagement and evaluation of the program.
**What you will be doing**
- Negotiate referral pathways internally and externally to ensure people who meet eligibility criteria are identified and supported to access the program
- Promote the program via a variety of channels
- Screen for eligibility and undertake client assessments for the program
- An ability to work sensitively with people of diverse cultures, abilities, ages, sexualities and gender identities
- Assist in volunteer recruitment, induction and training
- Assist colleagues in matching volunteers to clients
- Lead evaluation of the program
**What you will bring**
- Key Selection Criteria_
- Tertiary qualifications in social work, or related discipline
- Minimum of 4 years' experience in a community or social work role within a community organisation
- Proven experience empowering adults to improve health and wellbeing outcomes particularly within diverse and disadvantaged communities
- An understanding of mental health and trauma informed practice
- Demonstrated experience in stakeholder engagement with proven ability to strategically engage, negotiate and influence, others to achieve outcomes
- Demonstrated ability to work in a team environment
- Effective time management and organisational skills
