We are seeking a highly skilled Administrator/Scheduler to join our team. This role is responsible for conducting client care assessments and care planning, managing client compliance, intake processing, clinical care planning, and reporting.
Key Responsibilities:- Conduct thorough client care assessments and develop effective care plans.
- Ensure seamless client compliance and efficiently manage client caseloads.
- Process intakes, create comprehensive clinical care plans, and provide detailed reports.
- Cultivate high-quality person-centered care that aligns with clients' needs and service/care plans.
- Assist with general administrative tasks.
- Participate in on-call duties with a rotating roster among the care management team.
- Respond to customer inquiries via phone and walk-in services.
- Certificate 3 in Aged Care/Individual Support (required).
- Demonstrated possession of a valid Driver Licence (required).
- Previous experience in aged care, NDIS, or mental health care coordination roles.
- In-depth knowledge of My Aged Care, Home Care Package, and NDIS.
- Enrolled Nurse or Registered Nurse (advantageous).
- Essential first aid and CPR certification.
- A current valid Driver's Licence.
- High level of computer literacy with expertise in Microsoft Office.
- An outgoing personality with high energy levels, excellent communication skills, and a personable telephone manner.
- The ability to think critically and solve problems quickly under pressure.
- A positive 'can-do' attitude and capacity to work effectively within a team environment.
- Multitasking with strong attention to detail while maintaining a calm and focused approach.
- Exceptional networking, consultation, and time management skills.
- Outstanding customer service skills with exceptional verbal and written communication abilities.
- In-depth knowledge and understanding of home care services and guidelines.
