Mission Australia Brisbane Homelessness Service offers a unique opportunity to work with individuals experiencing homelessness.

As a Case Manager, you will provide support and guidance to clients within various settings, including residential and other accommodation types. You will work closely with clients to develop individualized plans to help them achieve their goals, update records, and communicate effectively with stakeholders.

Your key responsibilities include:

  • Case managing up to 20 clients with complex needs
  • Developing personalized support plans to help clients achieve their objectives
  • Updating records, filing notes, and generating reports for the service
  • Inducting new clients into the service and ensuring they are aware of relevant policies and procedures
  • Liaising with a diverse range of stakeholders, including clients, community organizations, health services, and government agencies

Requirements for Success

We seek an individual with:

  • Diploma or degree qualifications in a relevant field, such as Social Work, Human Services, or Psychology
  • Relevant experience working in a similar role, ideally in a case management environment with homeless clients
  • Strong skills in advocacy, communication, and referral
  • The ability to build positive relationships with clients who may present with complex behaviors
  • Sound computer skills, particularly in MS Office, and strong written communication abilities
  • A current First Aid Certificate and a valid driver's license
