The Chronic Disease Monitoring Service Team Leader plays a vital role in upholding our core values of being the best, a role model, open and honest, compassionate, respectful, and collaborative.
We aim to enhance chronic disease management and prevent hospital admissions by conducting thorough assessments and providing suitable devices to participants based on their digital literacy and condition severity. Participants will be monitored remotely daily by the monitoring service team.
The Team Leader will oversee a team of Registered Nurses and Allied Health professionals with Medical support, collaborating with key stakeholders to implement the new model of care considering clinical risk, inclusion criteria, intake processes, and clinical escalation processes.
Responsibilities include implementing the agreed model of care, recruiting and selecting delegated staff, defining roles and responsibilities within the team, providing line management and supervision, and working in partnership with other clinicians and managers to integrate services and streamline patient journeys.
