Case Manager Position

We are seeking a dedicated and compassionate individual to join our team as a Case Manager. As a Case Manager, you will work directly with individuals and families experiencing or at risk of homelessness, providing crisis intervention, information, referrals, and ongoing support.

Key Responsibilities:
  • Conduct client intakes, assessments, and develop case plans.
  • Provide crisis intervention, information, referrals, and ongoing support.
  • Advocate on behalf of clients with government agencies, courts, and service providers.
  • Undertake home visits with clients in temporary or transitional accommodation.
  • Facilitate connections to life skills and education programs.
  • Maintain accurate client records and case notes.
  • Create a safe and welcoming environment for all service users.
  • Build partnerships with community services and stakeholders.
  • Support service delivery targets and contribute to continuous improvement.
Requirements:
  • A minimum Certificate IV (or current tertiary study) in welfare, community services, or a related field.
  • Experience working with people who are homeless or disadvantaged.
  • Strong understanding of issues including family breakdown, mental illness, addiction, and social disadvantage.
  • Knowledge of local community support services and ability to foster strong partnerships.
  • Excellent written and verbal communication skills.
  • Solid computer and administrative skills.
  • Ability to work independently as well as collaboratively in a team.
  • A current NSW driver licence.
